Since 1908 - honorary life surgeon of the Imperial Court. Under his leadership, a new obstetrics and gynecology clinic was created at the Military Medical Academy, which he headed for 10 years. Since 1905, he actively participated in public political life. Member of the 2nd and 4th State Dumas from the Volyn provinces; belonged to the Octobrist faction. Since 1908 - Chairman of the Medical Council of the Ministry of Internal Affairs - the highest medical authority in Russia. Since March 1915 - member of the State Council. In 1916 - 1917 - Chief Administrator of State Health Care (as a minister). After the February Revolution of 1917, he was arrested, kept in the Tauride Palace, and then transferred to the Peter and Paul Fortress, soon released, and participated in the White movement. He emigrated to Bulgaria, where he took the position of professor of gynecology at the Faculty of Medicine of Sofia University.
